About the Program

Annual Check UpThe Neighborhood Health Center (NHC) clinics were established to provide family medicine services and resources to the Greater Houston’s uninsured and underinsured patient population. The NHCs work closely with area Memorial Hermann Hospitals to accept and treat patients presenting to the emergency departments. The primary goals are for patients to establish a primary care provider with the NHC and encourage and teach the patient population to maintain wellness and good health.

Services Offered

Both walk-in and scheduled appointments are available to the patients. The NHCs are staffed by a physician and nurse practitioners who provide quality of care services including wellness exams, well-child exams, health and sport physicals, vaccinations, immunizations, minor procedures, joint injections, diagnostic laboratory services and nutritional consultations. The NHCs are open seven days with extended and convenient hours of operation.

Program Goals

The ultimate goal of the Neighborhood Health Center is to be a medical home for our patient population as most of the patients seen realize the NHCs are their only option for medical care. The NHCs charge $55 a visit with attention to affordable lab tests and prescriptions, provide and promote coordination of care with other providers, and conduct voluntary patient surveys to identify patient needs in regards to food insecurity, housing, economic barriers and healthcare support. Neighborhood Health Centers have implemented the Exercise is Medicine program to encourage and acknowledge exercise and wellness within the patient population.
